nobu 1e3a1bbb66 ruby.h: add prefix ...

* include/ruby/ruby.h: prefix RUBY or RB to global symbols to get
  rid of name conflicts with other headers.
* include/ruby/encoding.h, include/ruby/intern.h: ditto.
